DIABETES SELF-MANAGEMENT

A 12-session Spanish language program for adults with type 2 diabetes and their family members.
DIABETES SELF MANAGEMENT
BRINGING OUR GLUCOSE LEVELS UNDER CONTROL!
Latino Health Access has garnered national recognition for its offensive against diabetes, a disease that affects one in five Latino adults.
Our strategy begins with promotores , or volunteer community health workers, who recruit participants for our program in laundromats, churches ,apartment complexes and community events. Potential participants may be more inclined to participate because it is inessence their neighbors recruiting them into the activity.
Participants take part in 12 weeks of peer-led instruction. Instructors are certified with a specialized curriculum and work with medical professionals. Participants are linked with professional health care providers. We challenge participants to role play, participate in demonstrations and share experiences. At each session, they measure blood glucose levels and body weight.
The results have been amazing!
We are so proud of our participants and promotoras for their hard work. !Gracias!
